It is believed a vehicle heading west lost control and collided with a vehicle which was heading east at the time.
The east-bound vehicle ran off the road and rolled down an embankment. The west-bound vehicle received significant front end damage.
A third vehicle tried to avoid the accident but was unable to pull up in time.
Three people in two of the vehicles were conveyed to Lismore Base Hospital for treatment.
The occupant of the third vehicle was not injured, and was able to drive from the scene after assisting police with their enquiries.
The Bruxner Highway was closed for a couple of hours while emergency services investigated the accident.
