The cars collided at Campbelltown Road near Rose Payten Drive about 7.15pm on Friday.
Two of the vehicles caught alight – A blue commodore driven by a 26-year-old man from Mount Annan and a Kia Sportage.
It is believed four occupants of the Kia Sportage, the car the 18-year-old man who died was a passenger in, were all seriously injured.
Four underwent surgery overnight, but are now in a serious but stable condition.
The 26-year-old driver of the blue Commodore was rescued by witnesses to the crash and underwent surgery at Liverpool Hospital for internal injuries, police said.
NSW Police are expected to address the media at 11:30am on Saturday, however investigations are expected to take some time due to “complexity of the crash”, a police spokeswoman said.
